Australians and New Zealanders throughout the world stood still for their national war memorial days in remembrance of the failed Australian and New Zealand Army Corps — ANZAC — attack on Gallipoli, Turkey that began on 25 April 1915. The fateful attack was designed to end the First World War more quickly by creating a supply line to Russia. A hundred-thousand died in the battle, remembered every year as ANZAC Day by both nations.

The British-directed battle of Gallipoli is often seen as the defining moment in the ‘birth’ of Australia and New Zealand. With New Zealand Prime Minister Helen Clark saying “For New Zealand as for Australia it was at Gallipoli that our young nations came of age.” [1]. This being the 90th anniversary of the attack, Clark, Australian Prime Minister John Howard and Britain’s Prince Charles are all at Gallipoli to remember that fateful campaign.

Some controversy has been created about Australian Prime Minister John Howard not attending the New Zealand ceremony at Chunuk Bair on the Gallipoli Peninsula. This has upset many people as it is a break in a tradition that the Prime Ministers attend the ceremonies of both countries.

Medical uniforms have been a professional requirement of health care workers starting as early as the middle of the 19th century. By 1880, Florence Nightingale had established a system to train nurses and used a hat and band system to identify nurses of different rank. Nurse hats originally were modeled after nun’s coifs, which gave the nursing uniform an increased look of respectability.

The First World War brought about a change in nursing uniforms to allow increased functionality in order to provide fast and efficient care to the large number of casualties of war. Bulky aprons were discarded and skirts were shortened. This change led to a post war period where nursing uniforms begin to mimic popular fashion.

In the 1950’s, hats began to be de-emphasized in order to make the nursing uniform less feminine and therefore attract more male nursing trainees. By the 1970’s the hat had disappeared almost completely except for use in nursing training institutions. The new trend in nursing fashion became uniform scrubs. Today, at most hospitals everyone wears uniform scrubs at all times to prevent the spread of infectious diseases.

Cherokee Scrubs arrived on the market with new styles to flatter the body and make scrubs less unisex fitting. Cherokee Scrub Pants came in a variety of styles to compliment different female body shapes, as well as the original men’s unisex styles.

Uniform scrub pants also come in regular, petite and tall lengths. The standard size range was expanded to include XP to 5XL in order to accommodate all healthcare workers who were required to wear uniform scrubs. Hospital often differentiated departments through the use of specific colors. Cherokee Scrubs continues to follow fashion trends and colors to provide healthcare workers with a comfortable, easily laundered and fashionable wardrobe.

I dont understand the trend to demean the wearing of uniforms. This trend does not just apply to the Nursing Industry, but extends to the vast number of business categories where a look is appropriate.

Consider the airline pilot. Would we be a happy traveler if we boarded a plane, looked into the cockpit (if indeed that was still possible) and saw our pilot clad in jeans and a T-shirt? Somehow, that stiff, crisp uniform with shiny buttons, white shirt, and most of all the captains hat fills us with confidence that our flight crew is indeed trained and certified to fly the airplane.

The white shirt on our bank President fulfills the same purpose as the airline pilots uniform. It inspires confidence in his ability to manage not only the banks money, but ours as well. Would we invest our hard earned dollars with someone sitting behind a desk in clothes that look like they just did a days gardening chores?

The nursess uniform is similar in nature. The health care uniform, whether it be a traditional white uniform or scrubs, is a symbol of knowledge. It represents the long hours studying anatomy or meds, the hours spent in clinical practice, the numerous nights spent cramming for tests. Symbolically, the knowledge learned is represented by the capping ceremony and graduation.

The uniform worn by a nurse is their badge of knowledge. It is the physical representation of years of studying and work that went into achieving that precious nursing degree. So why would a nurse not want to be recognized as a professional healthcare giver? Why not wear a uniform that sets them apart from the general public? Why not wear it proudly..

Canadian PM and Quebec premier announce plans for highway development in Montreal

The PM and Quebec premier Jean Charest have announced a $1 billion highway development in Montreal. This project would be one of Canada’s largest ever public private partnerships. It will be a 35 kilometer (21.8 mile) four-lane toll road Highway 30 project, in Montreal.

“It opens up a new horizon in Quebec’s economic development,” Stephen Harper said in a speech in Lery, Quebec. “As prime minister of Canada I am very proud and welcome this latest example of the fruitful collaboration between our government and that of Quebec.”

“Since we formed the government of Canada, I have wanted to practice an open federalism, inspired by the spirit which gave birth to the Canadian confederation,” said Harper. “That includes respecting provincial jurisdictions and the sharing of responsibilities.”

“You know that open federalism is not a constitutional theory or an electoral slogan,” the prime minister added. “It’s a new approach that allows partners in the Canadian federation to work better together in the realization of projects.”

Completing the highway will allow through traffic to bypass Montreal, reducing traffic congestion and air pollution in the area.

“The announcement today speaks to the willingness of both governments to work together to better serve the interests of the population,” said Charest, who is expected to head into an election next year.

Bank of Canada Governor David Dodge and other public officials have endorsed agreements between governments and private companies to invest in roads and hospitals.

Hungarian state-owned enterprise acquires Hirtenberger Defence Group

On Monday, Hungarian daily newspaper Magyar Nemzet revealed a deal whereby, on October 29, Hungarian state-owned enterprise HDT Védelmi Ipari Kft. (HDT Limited) acquired Austrian mortar and shell manufacturer Hirtenberger Defence Group. Hirtenberger Defence Group also acknowledged the deal.

In the deal, Austrian, United Kingdom and New Zealand production sites are to be kept, with market sales expected to cover for the price in the long run. A government-guaranteed loan covered the costs, but the exact figure was kept secret.

The November 4 issue of Magyar Nemzet published an interview with Gáspár Maróth, the Hungarian Government commissioner responsible for defence, defence industries and coordinating defence modernization, in which the deal was announced. On the same day, the Hirtenberger Defence Group issued a corresponding press release.

While not naming any other bidders, Maróth said the Hungarian bid won after lengthy negotiations, out of mulitple competing offers. He said he expects no political turmoil between the neighboring Austria and Hungary over the sale. He also noted the acquisition fits into the Hungarian “outward investment” ((hu))Hungarian language: ?t?kekihelyezési strategy of Prime Minister Viktor Orbán.

The Austrian partner asked for the price to be kept secret, but the commissioner stated the purchase was financed by a loan, so it is not a burden on the state budget. With government resolution No. 1430/2019, the state issued an indemnity bond to cover for HDT’s 38.8 million euro loan, and daily Népszava covered the story with a headline pointing out that amount as the presumed price. The creditor Hungarian Development Bank is state-owned as well. Already-queued orders won’t leave idle capacity for some years, Maróth said, and paying back this loan seems realistic; unnamed industry experts called this dubious to Népszava, as the market is saturated and margins are thin, while noting this kind of equipment is needed by the Defence Forces.

The company structure remains unchanged and all sites continue to operate, but Hungarian engineers are to join the research and development team. Maróth pointed out it would have been much more costly and time-consuming to research mortar technology than to acquire it this way. Népszava noted the purchase appeared to circumvent the Ministry of Defence. HDT Limited was registered on August 6, 2019 and is under the aegis of Hungarian National Asset Management, Inc.

To modernize its military, in 2018, Hungary licensed Czech firearm technology and started assembling firearms in Kiskunfélegyháza, then agreed with Airbus to open a helicopter parts factory as a joint venture in Gyula.

US President Trump has called for NATO members to increase military spending. According to a March 14, 2019 NATO press release, Hungary spent an estimated 1.15% of its GDP on military expenditures in 2018; NATO guidelines call for 2%. Maróth noted the military had been “effectively disarmed” ((hu))Hungarian language: ?gyakorlatilag lefegyverzett by 2010, when Viktor Orbán regained the prime ministry of Hungary for his second term. Hungary launched a military development program dubbed “Zrínyi 2026” in 2017 to address structural and financial challenges faced by the Defence Force and to renew its equipment.

Hong Kong ‘tutor king’ applies for bankruptcy

Hong Kong ‘tutor king’ Karson Oten Fan Karno, known to the city by the pseudonym of ‘K. OTen’, applied for bankruptcy following a failed appeal. He was sued by the tutorial schools Modern Education and King’s Glory Education Centre for breach of contract and was sued for $8.87 million and $26 million respectively.

Karno is a well-known but controversial English tutor who taught under Modern Education and King’s Glory who claimed to have 9,000 students at the peak of his career. In his notes he used words which some parents considered coarse, and he reportedly used profanity in class. Modern Education accused him of printing large advertisements on buses without authorisation and lying by saying that he was the chief English tutor. He has also been accused previously of copyright infringement, but was not charged. A former student said that he ‘used slang in class, taught dating skills in notes, and was just like the students’ friend.’

Karno said that he is not only his family’s pillar, but is also a father. He explained that he could not afford to pay for lawyers while fighting alone against a large corporation. He decided to stake his reputation for the sake of his family by applying for bankruptcy. On Wednesday, he signed a contract with Contab Education as the chief English tutor and would continue to provide lessons twice a week. He also urged King’s Glory to patch up the monetary quarrels and strive to improve the city’s education instead.

A tutorial school sector worker stated that the application was expected, but feared that the incident will make the students think that one does not have to be responsible for one’s own actions. He went on to say that the incident may give the impression that tutors cannot be trusted, and that the image of the tutorial industry may be greatly affected.

The IAPAM (International Association for Physicians in Aesthetic Medicine) offers the following information for aesthetic physicians on some alarming and illegal trends dealing with cosmetic injectable treatments, specifically DIY (Do It Yourself) Botox treatments and illegally imported botulinum toxin (Dysport, Botox) product.

The Recent DIY Botox Trend

Recently, there have been several reports regarding DIY “botox-like” injectables, which can be purchased through the internet. A woman in Texas, Laurie D’Alleva, offered consumers a botox-like product called “Freeze,” complete with a “How-To” video, so consumers could administer the botulinum toxin themselves. “The Texas Attorney General is charging D’Alleva with several violations of state law. She could be fined up to $25,000 per violation per day of the Texas Food, Drug and Cosmetics Act, and up to $20,000 per violation of the Texas Deceptive Trade Practices act. The red-flags regarding this internet offering range from the unrealistically low pricing, indicating the product is not FDA approved, to her complete disregard for the potential life threatening side affects that can accompany the delivery of a neurotoxin into the body by an untrained, unlicensed administrator.

Cosmetic Injectables Must Be Purchased in US

A parallel issue with the administration of botox by non-physicians is that “Botox and Dysport and all other cosmetic injectables can only be sold legally in the US …. to physicians.” These products must be legally approved and labeled by the FDA. Depending on state law, which varies, only physicians, nurse practitioners, and nurses may inject these substances.

Dr. Russel Kridel offers his expertise and concerns regarding the issues associated with purchasing cosmetic injectables. “The products obtained must come from the US where the FDA maintains safety levels not so strictly adhered to in other countries. Importation of cosmetic injectables can be a felony, subject to one year in prison and $100,000 in fines. Recently 5 physicians, a nurse and a practice manager in New York plead guilty to such charges. Therefore patients p[and doctors, alike] need to be cautious when injectables are offered at bargain prices, because they may have been obtained through web sites or offshore; they may be counterfeit and there s no one to assure purity or content. Serious injury or personal harm may result when unknown substances are injected.”

Dr. Melissa Babcock also generously shares her insights regarding the purchase of botulinum toxin products. “It is true that Allergan, the maker of Botox, sells its product to pharmacies outside the US for a much reduced price. I have received flyers in my office offering Botox for up to 30% off from Canadian pharmacies and other internet sources. I have know of physicians buying Botox from Canada. It certainly is enticing to receive a 30% discount on an expensive injectable.

I bought Botox from a Canadian pharmacy. When the product arrived it was not Botox Cosmetic but regular Botox. The two products are identical. When Allergan applied for FDA approval of their botulinum toxin to be used for cosmetic reasons the FDA approved the drug but required Allergan to add the word “Cosmetic” to the packaging. This concerned me, but my other concern it that it took 7 days for the product to arrive. Normally when I order Botox from Allergan I receive the product in less then 24 hours and on dry ice. Botox is extremely sensitive to re-warming. Re-warming would certainly decrease the efficacy of the product.

Before using the Canadian Botox I called the Georgia Drug Association to inquire. A representative told me it was illegal to dispense or to treat patients with medications from Canada. Specifically every drug manufacturer who wishes to sell medications in the state of Georgia must have a license. The Canadian companies do not. The penalties for using medications purchased from Canadian pharmacies are hefty fines and revocation of the physician’s medical license. Since that time I have only ordered Botox from Allergan and I will never order medications or even sunscreen from companies outside the US.

If you are a patient and wonder if you physician is using Botox purchased outside the US first you should ask if they are using Botox Cosmetic. The word “cosmetic” is a hologram on the bottle. Canadian Botox is usually not Botox Cosmetic. When you make your appointment, tell the staff that you want to see the bottle when you have your injection. If the office does not want to show you the bottle then go elsewhere. Additionally every bottle has a lot number. When you call for your appointment you could ask to see the lot number on the bottle. This number would identify whether the Botox was sold in the US.”

Medical Malpractice Insurance Coverage Could be at Risk

Susan Preston, President of the Professional Program Insurance Brokerage firm is very clear about the implications to a practice if a physician purchases botulium toxin products from an unlicensed distributor or another country. Many, if not most insurance carriers, including ours, only provide coverage for Botox or other injectable products bought in the United States. There have been reports of people dying when treated with an injectable product from another country. The risk is too great when buying products from other countries as no one controls drugs as well as the FDA does in the United States. Also the manufacturer might not support liability for imported products especially if they were altered in any way. Thus the person getting the injectable solution cannot be sure that it is manufactured to US specifications, backed by the manufacturer and the potential for major injury including death is much greater.

Cosmetic Treatments Should be Performed in a Medical Facility

Dr. Kridel shares that “doctors offices or other medical facilities are the safest place to have injectables administered,” either during botox training or during patient treatment….Not too many spas, peoples homes, or hotel rooms have the same standards for cleanliness and emergency care that a doctor s office does!”

The FDA agrees, and specifically states that, “botulinum toxin products should be administered in an appropriate setting using sterile instruments. Malls, private homes, [hotel rooms, and conference rooms] are not medical environments and may be unsanitary.”

Furthermore, Dr. Kridel reminds practitioners that, “all staff should be trained in sterile procedures so as to avoid contamination, re-use of needles, and infections. Plus, in the rare case of an allergic reaction, only qualified staff and physicians are trained in resuscitative measures and have emergency drugs available.

Comprehensive Botox Training Needed

Dr. Melissa Babcock reinforces for physicians and patients alike, the importance of having a skilled professional involved in product preparation. “If you are not a trained medical professional, botulinum toxin (Botox or Dysport) can be very dangerous to use. Medical professionals are trained to dilute the concentrated product correctly, inject it correctly into appropriate muscles and use the correct concentration for each muscle they are injecting. An untrained person injecting Botox would certainly experience side effects such as drooping eyelids, and other facial distortions resembling a person who suffered a stroke.

Dr. Thomas Sterry also champions the importance of being trained in a product’s preparation and dilution. “Botox comes in 100 unit vials that need to be diluted by the physician. Some doctors are accustomed to diluting it with 10 cc s of saline, making the concentration weak, but they inject more volume. Some make a very concentrated solution to give more pin point results. It s a style point with no generally accepted right way to do it, but the more concentrated solution does tend to hurt less.”

The Patient Consultation Is Central to a Successful Outcome

After training, managing patient expectations is the next most critical success factor in a good outcome with botox. Dr. Sterry offers these tips for physicians to discuss with their patients:

a. Ensure that you discuss with the patient, “what they are paying for.” Some physicians will charge per area (e.g. forehead) while others will charge based on how much product is being injected. b. Ensure the patient understands the office policy for touch ups before beginning the treatments. Some offices charge, while others will not. Typically, when patients pay by the unit, touch-ups are not “included.” c. Council patients that they should wait at least three months between injections to avoid the risk of developing antibodies to the botox, which will counteract the effects of injections.

Conclusion

Dr. Sterry provides these final thoughts. “People love Botox – patients and doctors – because of how accessible it is and for how minimally invasive it is, [but] the ease that people perceive it’s administered with has become exaggerated. In order to administer injectables with the best results, one must know very well the layout of facial muscles and understand well the product being injected. Finally, a huge part of injectables is artistic flair – you are using these products to literally fine tune the physical appearance of the face in a manner that is semi-permanent. Whatever the result is, it’ll be there for some time.”

“The most important thing for both patients and physicians to keep in mind is that injecting the skin with Botox, Dysport, Juvederm or any other product is not ‘easy’ and even otherwise trained professionals have seen far less than stellar results. A colleague of mine let me know that even after undergoing some training with Botox injections, he was still unable to generate the results he wanted. In one case, he very unfortunately had some trouble with the muscles around the eye. Certainly after more Botox training and practice he’s honed his skill but it just goes to show that these injections must not be taken lightly [just] because they are far less invasive and less expensive than other facial cosmetic procedures.”

The US economy added 173,000 jobs in August, the Bureau of Labor Statistics reported on Friday. The unemployment rate fell from 5.3 to 5.1 percent, the lowest since April 2008.

Although August job gains were lower than most economists forecast, job growth numbers for June and July were revised upwards by a combined 44,000. Average job gains over the past three months stand at 221,000, compared to March-May’s 189,000 monthly average. Over the past twelve months, job growth has averaged 247,000 per month.

Average hourly earnings rose 0.3 percent, or 8 cents, marking the largest increase in earnings in seven months. Hourly earnings had risen by 6 cents in July. Wages have risen by 2.2 percent over the past year.

Job growth in August was primarily concentrated in the health care and social assistance, financial activities, and professional and business services sectors. Those three areas of the economy added a combined 108,000 jobs. Food service and drinking places employment increased by 26,000 over the month, and other economic sectors saw employment hold steady. Manufacturing, on the other hand, saw employment decline by 17,000 in August. A stronger dollar and worldwide economic weakness make US exports less desirable, leading to a flattening in manufacturing employment so far this year after steadily rising in the early years of the US economic recovery.

The solid overall job gains led analysts to slightly raise expectations for a decision by the Federal Reserve to raise interest rates this month. Investors raised the likelihood of a September rate increase from 26 percent before the jobs report to 30 percent, and stocks dropped by over one percent on Friday. “The payrolls data is certainly good enough to allow for a Fed rate hike in September,” said Deutsche Bank’s head of currency strategy, Alan Ruskin. “The big question is still whether financial market volatility will scupper the plans.”

“This is the first time the market has looked at a Fed meeting and really has no idea what the Fed is going to do,” said Mark Kepner, a New Jersey equity trader with Themis Trading. “Right now you’re looking at the overall uncertainty and that’s what’s hanging on the market. I don’t think this number in and of itself changes how somebody’s going to vote.”

Transport for London wins first Anti-Social Behaviour Order against graffiti vandal

Billy Murrell, a persistent graffiti vandal from South East London, has become the first recipient of an Anti-Social Behaviour Order (Asbo) granted to Transport for London (TfL) by Greenwich Magistrates. The civil order also bans him from the top deck of buses throughout England and Wales for three years.

Murrell, a 17-year-old from Plumstead, has a history of convictions for criminal damage on public transport, including vandalising a Tube carriage in Brixton station and for damaging buses and other public property using marker pens.

This is Transport for London’s first Anti-Social Behaviour Order against a graffiti vandal — TfL was granted the power to apply for Asbos by the Home Secretary in September 2006.

The Anti-Social Behaviour Order was issued at Greenwich Magistrates Court on 12 September and also bans him from carrying any permanent marker pens or any glass cutting equipment on London Underground, railway property or any other transport provider’s property.

Metropolitan and Transport police have been made aware of Murrell’s Asbo, and have distributed his photo.

In detail, Murrell is prohibited from:

  • Entering any depot, siding or other part of London Underground property or railway property or any transport providers property which is not expressly open to the public whether on payment or otherwise throughout England and Wales
  • Carrying the following articles, in any area specified (above) or in any public place, namely any form of unset paint in any form of container, any form of permanent marker pen, any form of shoe dye or permanent ink in any form of container, any form of paint stripper in any form of container, any form of grinding stone, glass cutting equipment, glass etching solution or paste, throughout England and Wales
  • Aiding, abetting, counselling or encourage any person who was attempting or committing any form of unlawful damage towards any property not belonging to or under the direct authorised control of the defendant throughout England and Wales
  • Travelling on the top deck of the any public transport bus within England and Wales

If without reasonable excuse the defendant does anything which he is prohibited from doing by this order, he shall be liable to a detention and training order, which has a maximum term of 24 months – 12 months of which is custodial and 12 months in the community

Upon turning 18 he will be liable to imprisonment up to five years.
